Sacramental Record Request Form
- Please allow up to 1 week after submitting government-issued ID to be able to pick up the Sacramental Record. Please allow for more time if the record is being mailed.
- Please use this form for requesting Sacramental Records and be sure to send a copy of your government-issued ID (ex. driver's license) to isabel@ststephencatholic.org.
- You must be the person that received the sacrament unless it's a minor.
- If the person is a minor the parent/guardian must be the requester and a copy of the government-issued ID (ex. driver's license) must be sent to isabel@ststephencatholic.org.
